MTGCD Well Registration Form(Revised)

This form can be used to register existing wells, new wells, transfer ownership of existing wells, and register a replacement well. If the well that is being registered is estimated to pump greater than 17 gallons per minute and/or is located on less than 10 acres of land, this application must be submitted with an operating permit. SEE RULES TAB FOR REGULATORY INFORMATION.

Operating Permit Application Form(Revised)

This form is used to apply for a permit to drill a well that is estimated to pump greater than 17 gallons per minute and/or is located on less than 10 acres. This mandatory form should is used in conjunction with the application for well registration form. A refundable deposit of $200.00 must be submitted with your permit application. This will be refunded to you within 30 days of MTGCD receipt of the state driller's report.

 This permit is subject to approval by the board of directors. No well may be drilled until approved. This permit must meet date sensitive timelines and is required to be notarized. IMPORTANT INFORMATION TO NOTE WHEN COMPLETING THE FORMS: Applications may be subject to inquire for additional information which may require you to get from a well driller. Incomplete permits can not be heard before the board of directors.
